2. Customer agrees to repair work at vehicle purchase location.
3. Certificate only good for term of 3 year HAIL SHIELD contract.
4. Customer must submit copy of repair receipt with this form.
5. Mail to: Hail Guard Network, Inc., P.O. Box 550482, Atlanta, GA 30355 and allow
15 business days for return check.
