Your signature on the bottom of this document confirms agreement to the terms and conditions of the
Hail Guard Network, Inc. Service Contract (“Program”).  Execution of this agreement means that the vehicles for the coverage and time period have been registered subject to the terms and conditions set forth below.  This is not an insurance policy but an annual service contract.  (Note: Print and sign 2 copies of this Dealer Service Contract)

Terms and Conditions

• Cost to Customer is a one time network fee of one thousand ($1,000.00) dollars plus twenty ($20.00) dollars per vehicle times the average yearly vehicle inventory per Customer location.   Payment is made via company check by next day delivery FedEX to the address of Hail Guard Network, Inc. as set forth above.
• Customer accepts Hail Guard Network, Inc. price guide matrix as cost basis guideline for Paintless Dent Repair (PDR).  Said price guide matrix for Paintless Dent Repair (PDR) can be found at the website for Hail Guard Network, Inc. and is subject to change without notice.
• Paintless Dent Repair known as PDR is the technique used to straighten dents without the need to repaint leaving the original paint unchanged.  Drill holes will not be made in vehicles without the customer’s prior approval.
• Hail Guard Network, Inc. agrees to respond to inspect damage within three (3) business days of hail damage that the Customer reports via telephone, e-mail, or facsimile.
• Customer agrees that Hail Guard Network is the first contractor to perform Paintless Dent Repair (PDR) on Customer’s hail damaged vehicle(s).
• In the event Paintless Dent Repair (PDR) per vehicle(s) does not exceed one thousand ($1,000.00) dollars or is a total loss Customer shall not be entitled to any refund or portion of a refund.
• Any vehicle sold by Customer prior to repairs for hail damage must be returned for Paintless Dent Repairs (PDR) within a reasonable time period to the Customer’s address for (PDR) repairs.  In the event the vehicle is not returned to Customer’s address above within a reasonable time then Customer forfeits coverage of the individual vehicle under this Program.  A reasonable time period shall not exceed fifteen (15) business days from the time of the notice of damage to Hail Guard Network, Inc. 
• Customer agrees to provide suitable space indoors or outdoors as requested by Hail Guard Network, Inc. and electricity at the address of the Customer as work space for Hail Guard Network, Inc. and its employees or subcontractors.
• In the event any vehicle(s’) panel is severely damaged to the extent listed as conventional repair (CR) using Paintless Dent Repair (PDR) price guide matrix customer agrees to forfeit Paintless Dent Repair (PDR) method per vehicle(s’) panel(s).
• This Program is not transferable without the approval of Hail Guard Network, Inc. in writing. 
• This plan has no cash value and is non-refundable.

Procedure for using the Program

• This Program activates the fifth (5th) day after receipt of the Service Contract and payment in full. 
• This agreement extends all other coverage on Customer’s vehicle(s). 
• Vehicle(s) must be damaged by hail only and at the contracted address of the Customer above. 
• Any covered vehicle(s) damaged by hail away from the said address will not be covered. 
• All repairs are Paintless Dent Repair (PDR). 
• The Customer agrees to contact Hail Guard Network, Inc. in writing via e-mail at the web site for Hail Guard Network, Inc. or facsimile number 404-881-6054 within five (5) days of any covered hail event at (address).

Coverage of Program

• The Program covers up to one thousand ($1,000.00) dollars of Paintless Dent Repair (PDR) per vehicle for hail damage to covered vehicles.
•  In the event that hail damage exceeds one thousand ($1,000.00) dollars per vehicle the Customer and Hail Guard Network, Inc. agree the price shall be set per Hail Guard Network, Inc. PDR Pricing Matrix. 
• Program does not cover parts, moldings or glass.
• Pre-existing damage shall not be covered.

Network Fee Per Location

• The one time network fee is one thousand ($1,000.00) dollars per location.              $1.000.00
• The vehicle fee is based on the yearly average inventory.________ x $20.00=      _______

I understand, acknowledge and agree to the terms and conditions set forth above and agree to maintain the covered vehicles in accordance with this Program.  The signature of an authorized representative for the Customer constitutes acceptance of the request for the above Service Contract.
